MEMBERSHIP

ALL memberships require a copy of medical records. (i.e., proof of vaccinations).

By Mail (for renewals and new members)
You may renew by downloading the membership form and sending it with your membership fee by mail.
You will then receive and email with our gate code and when you can pick up your tags at the park.
RULES & REGULATIONS

PLEASE READ CAREFULLY

1. This facility is for Anthem Pets, Inc. members only,
please have current tags visible.
2. Anthem Pets, Inc. reserves the right to revoke
membership.
3. Please keep dogs leashed until in the gate and
please
move away from gate upon entering.
4. No more than 3 dogs per handler.
5. No dogs under 4 months.
6. Please pick up after your dog
7. Please do not groom your dog
8. Owners must always be inside the fenced area
with their
dogs.
9. No dogs in heat are allowed.
10. Agressive dogs must be leashed and leave the
premises
11. Small dogs under 30 use Small Dog area
12 . Large dogs over 30 use Large Dog area
13. No smoking, alcohol or food in the area.
14. No children under 12, children 12-16 must be
accompanied by parent.
15. Children should not run, chase dogs, or play with
water
features or structures intended for dogs.
16. Owners are responsible to cover holes or turf
damage
caused by pet(s)
17. Owners take full liability for their dogs and upon
entering.

This is a desert terrain, enjoy with care!
 Anthem Pets Inc. is a non-profit organization in
order to operate and maintain the park in an
effective way the park is open to members
only.

YOU MUST FILL OUT THE APPLICATION and
provide vaccination records. We screen all our
pet members for safety of other pets and
people. The yearly membership fee for each
dog is $35.00. Memberships run from August
1 through July 31.

Once we receive your completed membership
form along with the required documents, you
will be sent an email with your personal access
code to the park, and when your tag(s) will be
can be pick up.
